Engineering the Agentic Infrastructure Behind Element5's Post-Acute Automation Platform

Post-acute healthcare operations ran on manual, brittle processes across eligibility, authorization, and revenue cycle workflows that couldn't scale. Ideas2IT built Element5's automation platform end-to-end on AgentHero, our own open-source agentic infrastructure layer. The platform is now trusted by 21 of the top 30 post-acute providers in the US, processing millions of automated tasks monthly.

01 Challenge

Post-acute healthcare operations ran on manual, disconnected processes across eligibility, prior authorization, and revenue cycle workflows. No enterprise-grade automation layer existed. Handwritten, scanned, and faxed records defeated rule-based systems, and every workflow gap reverted to human handling that couldn't scale with provider growth.

02 Solution

Ideas2IT built Element5's platform end-to-end on AgentHero, deploying its Execution, Decision, Monitoring, and HITL layers as the foundation. On top of that, the team built deterministic "agents-on-reins" control, LLM-powered document comprehension for unstructured clinical records, and a pre-built RCM workflow library covering eligibility, authorization, audits, and collections.

03 Outcome

Twenty-one of the top 30 post-acute providers in the US run their operations on the platform. Millions of automated tasks process monthly across eligibility, authorization, and RCM workflows. Pre-built modules cut deployment time for mission-critical workflows across the sector.

Phase 01

Establishing the infrastructure foundation before any healthcare workflow was built

AgentHero Deployment and Deterministic Control Layer

The first engineering decision determined what the entire platform could do: Element5's automation had to be deterministic before it could operate at enterprise scale in clinical RCM environments. Unconstrained model behavior was not acceptable in eligibility and authorization workflows where errors carry direct financial and compliance consequences.

Ideas2IT deployed AgentHero's Execution and Decision layers as the foundation, with Guardrails, HITL routing, and Audit and Trace components providing the control envelope. On top of that infrastructure, the team implemented the "agents-on-reins" architecture that bounded agent outputs to defined action spaces, making high-frequency RCM task execution reliable without human review at every step.

This phase produced
  • AI Gateway, Agentic Mesh, and Guardrails configured for post-acute RCM workflows
  • HITL routing and Uncertainty Quantification for clinical automation control
  • Tracing, Evaluation, Explainability, and Cost Controls across all agent operations
  • Deterministic "agents-on-reins" control architecture
  • Audit and Trace layer

Phase 02

Converting paper-based clinical records into automation-ready workflow inputs

LLM Document Comprehension and Pre-Built Workflow Library

Post-acute RCM workflows break on documents: handwritten physician notes, faxed prior authorization requests, and scanned eligibility forms that no rule-based system parses reliably. Ideas2IT built an LLM-powered document comprehension layer on AgentHero's Document Comprehension component, trained to extract structured data from this class of input without fallback to manual handling. On that foundation, the team built a pre-built healthcare library covering the four RCM domains where manual work was most acute: eligibility verification, prior authorization, audit support, and collections. Each module shipped ready to deploy, removing provider-side engineering overhead for standard workflow automation.

This phase produced
  • LLM-powered document comprehension engine
  • Structured data extraction pipeline
  • Eligibility verification automation module
  • Prior authorization automation module
  • Audit support automation module
  • Collections automation module

Phase 03

Packaging the platform for enterprise adoption across the US post-acute sector

Delivery and Scale

Reaching enterprise scale in post-acute care required a delivery model that removed implementation friction at the provider level. Ideas2IT packaged the platform with 24x7 monitoring via AgentHero's Monitoring layer and a continuous workflow update cycle that kept automation current without provider-side engineering involvement. The pre-built library structure meant new providers could go live on mission-critical RCM workflows without custom build cycles. The combination of AgentHero's infrastructure depth, deterministic control, and ready-made modules produced the adoption result: 21 of the top 30 post-acute providers in the US now run their operations on a platform built end-to-end by Ideas2IT.

This phase produced
  • 24x7 monitoring and operations layer
  • Continuous workflow update system
  • Pre-built module structure enabling fast onboarding
  • Enterprise adoption infrastructure

The Outcome

Category Metric Description
Adoption 21 of the Top 30 Platform adopted by the majority of the largest post-acute healthcare organizations in the United States.
Monthly Automated Tasks Millions Eligibility, authorization, and revenue cycle workflows processed continuously across live providers.
Pre-built RCM Domains 4 Eligibility verification, prior authorization, audit support, and collections modules ready for deployment.
Agentic Layers Deployed 5 Execution, Decision, UX, Monitoring, and Deployment layers built on the AgentHero platform foundation.

The adoption pattern across 21 of the top 30 post-acute providers reflects a specific infrastructure decision: building on AgentHero meant the control architecture, monitoring stack, and HITL routing were production-grade from day one, before a single healthcare workflow was built on top. Deterministic control made enterprise deployment safe. Pre-built modules made it fast. The SaaS delivery model made it repeatable. Ideas2IT built the infrastructure and the vertical product layer.
